A significant part of caring for the mentally ill patients during our mental health clinicals was sitting down and planning interventions with the patients- asking them directly the different problems they’re having at present and how we can address them together. The care plan was designed to be wholistic; it includes the environmental, physical, physio-pharmacological, social, and educational aspects of care. Through observations and interviews, I got to know the patients and formulated the specific action plan around their preferences as much as possible. Below is an example of one of those care plans that I designed with substantial input from the patient. A care plan is bound to be more meaningful and successful when it is more personal and when the patients are more involved.
